    The following sections are required to be read, understood and agreed to in order to be eligible for employment.
    This Background Check Consent Statement documents your consent for New Mexico Wineries, Inc. ("NMW") to conduct an investigation, from a consumer reporting agency,  into all factors that may bear upon my suitability for employment, including, but not limited to: social security number verifications; credit reports and history; criminal records and history; public court records; driving records; accident history; educational history verifications (e.g., dates of attendance, degrees obtained); personal and professional reference checks; professional licensing and certification checks; drug/alcohol testing results, and drug/alcohol history in violation of law and/or company policy; and other information bearing on your character, general reputation, personal characteristics, mode of living and credit standing.    NMW requires a background check for the following individuals: All applicants, excluding temporary and part-time positions.  HireRight, Inc. (“HireRight”), or another consumer reporting agency, will prepare or assemble the background reports for NMW.  HireRight, Inc. is located and can be contacted by mail at 5151 California, Irvine, CA 92617, and HireRight can be contacted by phone at (800) 400-2761.  Information about HireRight’s privacy practices is available at www.hireright.com/Privacy-Policy.aspx.    I understand that an offer of employment from NMW for any regular full-time position, excluding temporary full-time employment, is contingent on the receipt and evaluation of the background check report.  In order to facilitate the required background check, I will provide NMW my social security number, current address and phone number (if not currently on file with NMW).  Failure to provide consent of the required information will result in withdrawal of any offer of employment.    I understand that if NMW hires me, my consent will apply, and NMW may obtain follow-up background check reports at any time during the course of my employment with NMW in any position.  This consent will apply throughout my employment in any position with NMW to the extent permitted by law unless I specifically revoke this consent in writing.  Revocation of this consent shall constitute a “for cause” basis for immediate termination of my employment in any position with NMW.  I have carefully read and understand this Background Check Consent Statement and, by my checking the box below, consent to the release of the above mentioned information to NMW within the terms of this statement.  This Background Check Consent Statement in original, faxed, photocopied, or electronic form will be valid for any such reports that NMW may request.
    I understand that New Mexico Wineries, Inc. ("NMW"), maintains a drug-free workplace and that I will be required to undergo a post-offer drug screen, with the exception of part-time or temporary employment. Additionally, I understand that I may be subject to a drug and or drug/alcohol screening during the course of my employment under specified terms and conditions, and the results of such screening may be grounds for disqualifying me or terminating my employment. I hereby consent to such testing. I authorize the testing laboratory to release my test results to NMW and designated supervisors and managers on a need to know basis.  If there is a positive test, I understand that NMW may ask me to provide and I agree to provide information about any legal non prescription drugs and other drugs for which I have a prescription that I take routinely or have taken within the last thirty days. I understand that any communication I may have with the collection site personnel, testing laboratories does not create or imply any form of doctor/patient relationship.  I understand that any offer of employment or continued employment, is conditioned upon NMW’s decision that the results of my drug testing are satisfactory.  I acknowledge that a telephonic, electronic or photographic copy of this document shall be as valid as the original.  If selected for employment I will receive a drug-free workplace policy.   By checking the box below, I consent to testing by a doctor, medical center, hospital, laboratory or medically qualified personnel.
